Home Support

Home Support/Home Care is ideal for individuals interested in staying in their homes. Our goal is to help people continue enjoying the comfort of their own home while they are provided with professional care support. Support may come in a variety of ways which includes house maintenance duties, medication reminders or accompaniment to appointments/meetings.  Home Support can be designed and coordinated to meet individual and family needs.

There are several types of home support Excel Society offers; we provide options in Care Coordination, Personal Care, Home Support, Companionship and Nursing.

Individuals suitable for this service include:

  • Individual senior living on their own or family member needing more care than they can provide
  • Someone returning from hospital requiring extra support
  • OR Caregivers needing little extra help

Care Coordinator Service

  • In home assessment of personal needs and home environment to create a plan in consultation with the client
  • Design, implement, and monitor a care support plan to meet the needs of our client
  • Ongoing scheduled visits to ensure satisfaction of our client and evaluate care plan
  • Coordination of external services such as health professionals or medical supplies delivery
  • Assistance to navigate the health system and obtain information as necessary
  • Communication with family members at the direction of the client

Personal Care Services

  • Personal hygiene and grooming
  • Assistance with bathing, toileting, dressing, oral hygiene, skin care, catheter care, perineal care
  • Medication assistance
  • Assistance with exercise routines etc.

Home Support Services

  • Basic home cleaning services
  • Cleaning kitchens, bathrooms, windows
  • Vacuuming/mopping of floors
  • Taking out garbage/Recycle
  • Laundry services – personal and bedding
  • Basic pet care
  • Administration of snow shoveling and lawn care

Respite/Companion/Personal Support

  • Visitation at home or in hospital/DAL site
  • Accompaniment to appointments or outings
  • Supervision/Assistance with daily activities
  • Assistance with meal preparation and clean up
  • Emergency response system available (paid monthly by client)

Nursing Care

  • Medication administration and monitoring
  • Vital signs monitoring
  • Wound care and dressing
  • Catheter care
  • End of life (palliative) care
  • Tube feed
  • Oxygen therapy
