Intake and Assessment is the first point of contact for individuals and their families who are seeking supports. Through Person Centred approaches, the Intake Coordinator will work with the individual and their family to identify goals/ dreams, and required support levels. Intake will review funding eligibility by appropriate funding bodies and support services available to assist individuals in reaching their identified goals. Intake will then facilitate applications for funding to enable services to be put into place, on the behalf of the individual and their family to acquire appropriate supports.
